Submitting a Request

Your clients can submit a request via SmartForms via an easy 3-step process:

Step 1: Login to your SmartForms portal with your Microsoft Account

Step 2: Select a SmartForm from the list based on your request

Step 3: Fill in the required details and hit 'Submit'

Login to your SmartForms portal with your Microsoft Account​

You can use your Microsoft Account to login to your SmartForms portal.

Select a SmartForm from the list based on your request​

Once you have signed in, you will be directed to a page with a list of all the SmartForms that is available for you. You can select a SmartForm from the list based on the request you are about to submit.

Note

This article mostly covers the scenario where you are logged in as a standard user. If you have admin role, you may see some additional options which are clearly defined in this article.


Fill in the required details​

When you click on a SmartForm, you will be taken to the next screen where Pia will inform you that a Support Ticket will be created for your request which you can use to track the progress on your request.

Once you confirm your actions, Pia will setup the SmartForms for you. Click on "Next" to view the form.

The final action now is to fill out the details and click on "Submit" to submit your request.

If you need more time or details to complete your form, you can click on "Save Draft" which will save your progress. Your progress will be saved for the next 48 hours after which the request will expire.
